F25 Toilet Repair

My toilet is in desperate need of repair, but I'm not sure what make and model it is. It looks like the original installation, so I am hoping someone out there knows what the OEM toilet on a F25 was. The label on mine is completely illegible, and I can't find any marking on the toilet itself. I have attached a couple of photos in the hope someone can help me out.

Hopefully a rebuild kit is still available from the manufacturer.

It's a Groco model HE, though if a certain rebuild kit is used it becomes a model HF.

You can still get parts but just a few months ago I gave up on mine. Raritan has a kit where you reuse the china bowl part but everything else gets replaced. My Groco never was very reliable despite eating $70 rebuild kits like popcorn. I just replaced the whole base and pump.
