I have a 39 Express with round bumps in the bilge in about the same places as your picture shows. No metal showing. Clearly the outline of a stud that was glassed over when the bilge was laid-up.

Very low tech. The hose barb was the first fitting in the fuel line starting from the tank end. When I saw the crude I dug it out with a piece of wire. Had the same conditions on a yanmar 4jhe, starvation under load. Turned out to be a restriction in the fuel line between the tank and fuel filter. I have a 39 Express that had the same problem. I put a piece of flexible tubing into the hole, sealed the hull side with plumbers putty and then poured epoxy in from the deck side until the area around the tubing was filled. I replaced the shut-off cables on a 28 Freedom and my 39 Express, in both cases they were standard Yanmar Part numbers.
